What content can you put in a QR code?

A URL (most common), an email address, a phone number, text or a vCard contact card. Most smartphones can scan QR codes directly with the camera app. The shorter the URL, the simpler (and more robust) the QR code.

How large should the QR code be for reliable scanning?

For a printed A4 document, a QR code of at least 2×2 cm is reliably scannable at normal reading distance (30-50 cm). The tool lets you adjust the size. When in doubt: larger is better.
