Referral letter and summary for other providers
Write a referral letter or summary for GP, specialist or insurer as PDF letter. Redact names and ID numbers when creating an anonymised version for supervision or peer review. Use the PDF sign tool for the treating clinician's signature.
Consent forms and privacy statement
Mental health patient records fall under the strictest privacy legislation (GDPR, HIPAA). Use PDF consent forms with explicit informed consent for treatment, data storage and any use for supervision or research. Retain for at least 15 years.
